Adult Case Manager

"Mountain Home is seeking a full-time Adult Case Manager (CM) to provide case management to young mothers between the ages of 16-29 diagnosed with severe and disabling mental illness. The CM links clients to needed medical, social, educational, vocational, and recreational services and provides advocacy and support which increases a client’s independence in the community.

This position is full-time, 32 hours/week. The schedule for this position has some required hours but also allows for the hired individual to build a schedule that works for them. Our goal is to meet the needs of the families served as well as the hired individual. This position reports to the Lead Case Manager and does not supervise others. This position is based onsite at Mountain Home in Missoula, Montana. The role requires local travel (~ 50% of the time) in company vehicles. The hourly wage range for this position is $19.00 - $21.50 with the hired individual’s wage calculated using Mountain Home’s equitable wage matrix.
